After downloading the membership form. Please fill out and mail the completed form to Kendra Stevens, Fulton County Juvenile Court, 395 Pryor Street SW. If paying online thru PayPal, please follow up by clicking on the PayPal button. Once you receive your receipt, please email a copy of it to Kendra.Stevens@fultoncountyga.gov. You also have an option to mail it.
